Intention For Qadha Fast
Question: Is it necessary to make an intention for Qadha fast?
Answer: Yes, it is essential to make an intention when observing the Qadha fast. The intention should be made before the start of the fast (i.e. before the break of dawn of that particular day).
It is also necessary to stipulate, for which day you are making Qadha. This may be acheived by making an intention that you are keeping the Qadha of the 'first' or 'last' fast in your obligation.

Combining Fast
Question: I missed fast in Ramadan due to my menses. Can I fast in Shawwal with two intentions: Qadha and fulfilling the 6 days of Shawwal Sunnah fast?
Answer: The two fast cannot be combined in one intention. The Qadha fast should be observed separately from 6 fast of Shawwal.

Injection While Fasting
Question: Is it permissible to take an injection while fasting?
Answer: Taking an injection will not nullify the fast. [Ahsanul Fataawa V 4: P 434)

Accidently Swallows Water While Fasting
Question: Does the fast break if one accidently swallows water while making Wudhu?
Answer: While making Wudhu, if water accidentally went down a person’s throat while the person was aware that he was fasting, his fast will break. Qadha will be Wajib, but there will be no Kaffarah (The person will have fast one day after Ramadan as a compensation) {Shami (vol. 2 pg. 401)}
